standard form

y=3(x-1)^2+12 [First write the equation]

y=3(x-1)(x-1)+12 [You should foil the (x-1)^2 in the vertex form]

y=3(x^2-2x+1)+12 [ Next Distribute the 3]

y=3x^2-6x+15 [This is standard form, y=ax^2+bx+c]
